In some cases, a root canal treatment of a tooth is not enough to preserve it in the long term. This is the case when the root canals at the root tip divide again into many small canals. Then it is almost impossible to remove all nerve parts with conventional root treatment and to seal the tooth clean and bacteria-proof.
In order to save the tooth anyway, it is possible to remove precisely this part of the root.

Using a modern surgical microscope, an opening at least 2-3 millimeters deep is prepared at the tip of each root canal. All microscopic side canals and ramifications at the tip of the root, as well as diseased tissue, are removed.

After disinfection and drying, the root canals are closed and sealed completely bacteria-tight under the surgical microscope with a biocompatible (retrograde) closure starting from the root tip.

The jawbone regenerates completely and the tooth can be preserved in the long term.
